INTRODUCTION


Henoch-Schonlein purpura is a disease inwhich there are purple spots on the skin, joint pain, gastrointestinal problemsand glomerulonephritis (a type of kidney disease). Purpura, arthritis andabdominal pain are the "classic triad" of Henoch-Schonlein purpura(HSP). Henoch-Schoenlein purpura (HSP) is a self-limiting disease.

The condition receives its name from thetwo physicians who first described this condition namely John Schonlein andEduard Henoch. It is a small vessel vasculitis having immunoglobulin A (IgA),C3 and immune complex deposits in the arterioles, capillaries and in thevenules.

 

 

CAUSES & RISK FACTORS


The exact cause is not known.

Antistreptolysin O titres are raised inabout 20 to 50% of the cases. Multiple factor like some foods, drugs, bites ofthe insects may as well trigger the Henoch Schonlein purpura (HSP).

 

 

SYMPTOMS & SIGNS


Symptoms

Rash mainly in the early stages may bepresent. It is more common on the legs.

Pain in the abdomen

Vomiting

Pain in the joints

Passing blood in the stools

Edema or swelling of the scrotum

 

Signs

Tenderness of the abdomen

Intestinal bleeding

Palpable purpura

EPIDEMIOLOGY


Frequency

About 14 cases are seen per 100000schoolchildren. The syndrome is usually seen in children but can affect peopleof all ages.

      

Gender

It is more common in boys than in girls.Male to female ration is about 2:1.

Age

75% of the cases are from 2 to 11 years ofage. It can also occur in adults.

Mortalityand morbidity

About 5% of the cases suffering fromHenoch- Schonlein purpura (HSP) have symptoms of the condition. Less than 1% ofHenoch Schonlein purpura (HSP) develops end stage renal failure.

 

TREATMENT OPTIONS


There is no specific treatment. Most casesgo away on their own without treatment. The disease is self-limiting.

Medical treatment

Analgesics can be used for abdominal painand joint pain. Anti-inflammatory medications are used such as Aspirin,Ibuprofen.  Cortisone like prednisone orprednisolone is also used. Cyclophosphamide, azathioprine or mycophenolatemofetil is also used to suppress the immune system. Antibiotics are also used.Intravenous immunoglobulin (IVIG) is sometimes used.

Swelling of the face

Loss of appetite

Nausea

Vomiting

The large movements

The skin rash

Prognosis

It is self limiting disease and can getbetter with time.
